Overview: I am pleased to present the revised guidelines for the administration of Intravesical AntiCancer Therapy. Bladder Cancer ranks among the top 10 most prevalent cancers worldwide. This condition is less frequently observed in women compared to men, with a ratio of 3:1. Non-Muscle Invasive Bladder Cancer (NMIBC) represents approximately 75 to 85% of all bladder cancer diagnoses. Acknowledging that Intravesical Anti-Cancer Therapies reduce the risk of recurrence, these guidelines will be essential for healthcare professionals managing this patient group to ensure safe and effective treatment delivery. In this revised edition, which has been extensively updated, you will find guidelines that include recognising and managing complications. Fundamental to this is accurate patient assessment before treatment delivery. Examples of evidence-based assessment tools are provided in the appendices.

Overview: Over recent years, there has been a move towards local anaesthetic transperineal prostate (LATP) biopsy as a safer and more accurate procedure. The risk of infection is lower as the needle transverses the perineum and not the rectal mucosa, and use of local anaesthetic is safer than a general anaesthetic. It enables a more accurate targeting of suspected cancerous areas for biopsy enabling precise targeting of the prostate and has resulted in earlier and faster diagnosis. With expanded scope of practice, Nurses and Allied Health Professionals (AHP) are being trained to undertake LATP biopsy, increasing the workforce capacity to deliver the diagnostics for the Urgent Suspected Cancer (USC) prostate pathway. Expansion of the workforce’s (trainee) scope of practice to include LATP biopsies requires theoretical and practical training, with endorsement of competency from a registered health professional (assessor), qualified to assess their competency. Practical training and competency endorsement processes vary, with a lack of standardisation or processes. There are several options for theoretical training including accredited courses, certification and local bespoke training packages. This is a guideline to support best practice, however individual Trusts may need to make adaptations for local resourcing, ensuring patient safety is maintained (e.g. some Trusts may not have a separate trainer and assessor available). The aim of the LATP framework is to give an overview of the minimum training requirements of competency for the trainee to deliver LATP biopsies and should be used in conjunction with the LATP training and assessment documentation
